There is no unified application form for leave under the BiR. It is drawn up in free form with the obligatory indication:

  • Full name and position of the head of the organization;
  • Full name and position of the employee submitting the application;
  • title of the document (application);
  • requests for leave under B&R;
  • dates for which leave is granted (in accordance with sick leave);
  • requests for benefits (optional);
  • attachments to the application;
  • dates, full name and signature of the employee submitting the application.

When preparing your application, pay attention to the following nuances:

  • There is no need to indicate the phrase “maternity leave” in the application, since officially there is no such concept. Instead, indicate “maternity leave.”
  • It is advisable to indicate in the application the most convenient way to receive benefits under the BiR: in person or on a bank card.
  • You can draw up the application yourself or write it according to the sample provided by the employer.
  • It is possible to fill out the application by hand or on a computer, but in any case it must bear the handwritten signature of the employee going on maternity leave.
  • Usually a vacation application is written from the start date of sick leave, but not always. Depending on the circumstances, the employee may postpone the date of maternity leave.
  • If the employee did not write a statement or wrote it from a later date, the vacation should be postponed. Days before the vacation, the employee must be paid her regular salary.
  • When deciding to postpone maternity leave, a woman reduces it - according to labor law, maternity leave cannot be extended.

Application for recalculation of the billing period. If a worker, being on maternity leave, goes on maternity leave, then she did not work for the previous two years, which are needed to calculate benefits. It also happens that the period between decrees is less than two years. Under such circumstances, the law allows changing the years of calculation that went before the first leave under the B&R, when the employee received income in full.

Note! If this statement is missing, then calculation based on actual income will be applied, i.e. along the bottom bar.

If necessary, you can write an application to change years - this will allow you to receive a larger benefit.

This right can be exercised even if the employee spent at least a day on the first one or two years before the second maternity leave.
