How to fill out a waybill for a tractor, sample with example

The obligation of organizations using transport or providing transport services to issue waybills is stipulated by law. Purpose of the document

Using the waybill, records are kept of:

• actual working time of the driver;
• tractor fuel consumption, gas stations;
• tractor mileage during a shift or trip.

The obligation to issue waybills is established by Order of the Ministry of Transport dated September 18, 2008 No. 152. This order contains the necessary details that must be included in the waybill, and also stipulates the procedure for filling out waybills, including the tractor waybill.

Why does a tractor driver need a waybill?

According to Russian legislation, waybills must be used at any agricultural enterprise with any organizational and legal form, with the exception of peasant farms and individual entrepreneurs. However, their use is mandatory only if the tractor driver performs work related to cargo transportation (clause 2, clause 52 of the Methodological Recommendations of the Ministry of Agriculture of the Russian Federation on accounting for the costs of maintaining transport equipment).

To obtain a waybill in agriculture, two forms are used, namely: (click to expand)

• form 412-APK - applied to all agricultural producers operating in the Russian Federation;
• Form 68 - used in those regions that have a common border with Ukraine.

These forms must be filled out when performing work on special equipment such as tractors and excavators.

Basic data

The following information is indicated on the front side of the waybill:

1. waybill number. Sheets are numbered in gross order, starting from January 1 of the new year;
2. Name of the organization;
3. codes OKUD, OKPO;
4. date of registration of the sheet;
5. Full name of the tractor driver, personnel number;
6. information about the equipment, trailer, if the tractor works with a trailer;
7. Full name of the person responsible for the trailer and the foreman;
8. brigade number;
9. a note on permission to leave, date and signature of the foreman who authorized the departure;
10. signature of the driver who accepted the vehicle.

Tractor waybill form 68

Form 68 – is used on the territory of Ukraine, but since many regions are located very close to it, they simply have to use this form for successful relationships. In order to avoid disagreements in the documentation, Russian legislation decided to adopt this form for those regions that border Ukraine, namely for the Rostov, Voronezh and Belgorod regions.

The structure of Form 68 is very similar to Form 412-APK, therefore tax inspectors are very loyal to waybills that are issued according to Form 68 from agricultural producers. But still, they advise sticking to the form of the waybill, which is approved by the Ministry of Agriculture of the Russian Federation.

Filling out the task field

This field indicates data on the tasks that are assigned to the tractor driver during the work shift:

1. name of the service or full name of the specialist at whose disposal the tractor is placed;
2. place of dispatch and delivery of cargo;
3. the distance over which the cargo is transported;
4. cargo information;
5. the number of trips required to transport the cargo;
6. number of pieces and weight of cargo in tons.

If there were delays or downtime, appropriate marks are also placed.

In a separate field, a note is made about the issuance of fuel, its remains upon departure and return, as well as about refueling, if any. At the end of the shift, the tractor driver and foreman put their signatures, recording the return of the equipment to the garage.

Form 412-APK - when is it preferable to use it?

The form arose on the basis of Order No. 750 of the Ministry of Agriculture of the Russian Federation dated May 16, 2003. According to the specified document, this form must be filled out when transporting goods on a tractor.

Form 412-APK consists of 2 sections, located respectively on both sides of the sheet. On the front side indicate:

• transport customer;
• loading and unloading points;
• mileage that will have to be done during the flight;
• information about the property being transported;
• number of flights to be made;
• quantitative characteristics of the cargo.

It is necessary to have the signature of a technician confirming the serviceability of the machine and the confirming signature of the tractor driver.

Section 2 is entirely devoted to the actual results of the work and reveals:

• delivery and loading points;
• category of cargo, start time of loading and departure from destination;
• working hours of the tractor driver, crew composition for loading and unloading;
• performance indicators - ton-kilometers;
• level of compliance with standards, additional payments;
• volume of fuel to be written off according to the norm and actual consumption;
• time on the way.

The form is issued for each shift and submitted at the end of the working day. Information from the sheets should be collected in special sheets.

Filling the reverse side

On the back of the waybill, notes are made on the progress of work assignments. It is filled out by the tractor driver, the specialist at whose disposal the tractor came, and the accountant. The table indicates:

1. point of departure and destination of the cargo;
2. name of the cargo and its class;
3. time of departure and arrival of the tractor;
4. number of the customer team;
5. accounting work code;
6. units;
7. hours worked;
8. quantity of cargo transported;
9. production rate;
10. price;
11. amount to be paid;
12. number of completed standard shifts;
13. conventional hectares;
14. mileage;
15. fuel consumption.

Below the table the results of the work for the shift are indicated, and the signatures of the driver and foreman are placed.

Distinctive features of a travel package for a tractor

A tractor is a special wheeled or tracked type of transport that performs transport, earthmoving and other road construction work in agriculture independently or in combination with various stationary and mounted implements (devices). However, the driver is not always required to have a permit with him and provide it to traffic police officers, but only if the tractor is transporting cargo from one destination to another or simply moving along the road. In other cases, when the tractor operates in the field, performing specific agricultural activities, a permit is not required.

First part of the form: front side

This is the main part of the tractor driver’s primary document. It is most often filled out by the dispatcher. The driver fills out the columns that outline the progress of the task. At the top of the form you should write the main details of the company: address, OGRN, name. Full name and OGRNIP are indicated if the farmer is an individual entrepreneur and not a legal entity. The front side of the voucher includes the following information:

• who gets the special vehicle at its disposal;
• class and name of cargo;
• names of points from where and where the cargo is transported, the distance between them;
• the time when the tractor set off and when it arrived at the place, addresses of the points of departure and arrival;
• information about the tractor (garage and state number, make and registration number of the tractor);
• number of departures on a voyage with and without cargo;
• dimensions of the cargo being transported.

The foreman examines the car and makes a conclusion regarding its serviceability, deciding whether it can be allowed on the flight. As a result of his check, he enters the speedometer readings on the front side of the form. The tractor driver receiving the car also signs for its acceptance. Before going on a mission, the driver will be examined by a nurse. A detailed examination of his condition for alcohol intoxication or use of psychotropic substances is carried out using a breathalyzer. If everything is in order, the nurse puts her signature in the appropriate column, allowing the tractor driver to take the shift.

Second part of the form: reverse side

The stages of the driver's task are displayed here. After indicating the name and class of the cargo, fill in the following fields:

1. the time when the tractor set off and when it arrived at the place, addresses of the points of departure and arrival;
2. tonne-kilometer trip;
3. the time period spent by the driver on the trip;
4. information about the customer’s team;
5. names of points from where and where the cargo is transported, the distance between them;
6. the number of standard shifts worked by the driver and their relationship with conditional reference hectares;
7. wages accrued to the driver for the shift worked;
8. run length;
9. fuel consumption (in the waybill of the tractor 412 APK they indicate how much fuel was given to the tractor driver for the shift according to regulatory reference data and how much fuel was left upon arrival at the garage; thus, the amount of fuel saved or what had to be purchased on the road is calculated).
