Rules for compiling a list
The document header contains two columns:
- the left column indicates the name of the territorial office of Rospotrebnadzor to which this list will be sent;
- in the right column the name of the organization is written, as well as the full name of its director.
After this, indicate the title of the document, as well as its serial number and date of preparation.
Then there is a table containing the following information:
- serial number;
- the name of the department or workshop in which the employee works;
- the employee's position in accordance with the staffing table;
- harmful and dangerous factors;
- paragraph number of Order No. 302N;
- full last name, first name and patronymic of the employee;
- employee's date of birth;
- floor;
- length of service in a given position in a given organization;
- date of last medical examination.
At the bottom of the table, the director of the organization puts his signature.
The list must be compiled in three copies: one remains with the organization, the second is transferred to the territorial office of Rospotrebnadzor, and the third to the medical company.
Who should undergo periodic medical examinations
Let's get back to theory. The medical examination was carried out and completed correctly, if you always have at hand the order of the Ministry of Health and Social Development No. 302n and the changes that came into force with the publication of order No. 1032 of December 13, 2019, in them you can find out about the deadlines that depend on harmful and hazardous production factors in a specific workplace.
Therefore, we open, study and begin to analyze the procedure for conducting periodic medical examinations of employees at the enterprise.
The following categories of workers are sent for periodic medical examination:
- working with harmful or dangerous production factors;
- performing work that is specified in Appendix No. 2 of Order 302n;
- transport managers;
- involved in underground work;
- employed in organizations of the food industry, public catering and trade, water supply facilities, medical organizations and children's institutions;
- athletes;
- minors;
- are busy working with PCs more than 50 percent of the time.

List of contingent workers
In order to compile a list of employees who are subject to periodic medical examinations, you first need a staffing table, from the text of which you must take the following:
- Full names of employees;
- Structural subdivision;
- Job title;
- Date of Birth;
- Enrollment Date.
As you can see, it must include the profession or position of the employee according to the staffing table and harmful production factors, with mandatory reference to paragraph of the Appendix to Order No. 302n.
This list is approved by the head of the enterprise and must be sent to the local branch of Rospotrebnadzor within 10 days. This deadline must be observed, since it is regulated by clause 21 of the Procedure to Order 302n.
It should be sent either by registered mail with acknowledgment of receipt and inventory, or by hand, but then do not forget to put a receipt stamp on your copy of the covering letter.
Name list
Both the contingent list and the list of names can be developed simultaneously and in parallel, because the second is a symbiosis of the first list and the staffing table.
It must include:
- Full name and position of the employee;
- Name of the structural unit;
- Name of the harmful production factor or type of work with reference to paragraph of Order No. 302n;
- In addition, the year of birth and work experience are indicated.
It is approved by the head of the enterprise, but it is sent not to Rospotrebnadzor, but to a medical organization that will conduct periodic medical examinations.
Which employees are subject to inclusion in the name lists
The following categories of workers must undergo a medical examination and, therefore, be included in the lists of names:
- minor employees - an annual preliminary examination must be carried out;
- citizens who get a job with harmful and hazardous working conditions are subject to preliminary and periodic examinations; until the citizen reaches twenty-one years of age, the inspection is carried out every year;
- persons applying for a job that involves the movement of transport undergo preliminary and periodic examinations;
- citizens working in the food industry, children's, medical and other institutions - preliminary and periodic examinations;
- Persons working in high-risk conditions undergo psychiatric examinations every five years.
What are the penalties for violation?
A list of employees for mandatory medical examinations must be compiled at the enterprise and submitted to Rospotrebnadzor. If this is not done, the following fines are imposed for violation of the law:
- for officials - from 500 to 1000 rubles;
- for individual entrepreneurs - from 500 to 1000 rubles or administrative suspension of activities for up to 90 days;
- for legal entities - from 10,000 to 20,000 rubles or administrative suspension of activities for up to 90 days.

Inspection schedule
The medical institution will draw up a calendar plan for conducting a medical examination. It must do this within ten days from the moment it received the list of names from the employer (but no later than fourteen days before the start of the inspection).
The medical organization must agree on the examination schedule with the employer or his representative. After this, the medical director approves. institutions.

What will be included in the list of contingents and lists of medical examinations.
The form of the list of contingents and lists for medical examinations is not approved by law. Recommendations on its content are specified in Order No. 302n.
So, the list of contingents must include:
- information about the employee’s profession, the name of which must correspond to the staffing table;
- data on production factors influencing personnel, containing references to the requirements of the above Order.
Lists of names are formed on the basis of a list of contingents, and include the employee’s passport data, the name of his profession, the production factor affecting him, and the structural unit to which the employee belongs.
The list of contingents is subject to development and approval within the enterprise once and is changed in the event of changes associated with changes in the number of harmful production factors, workplaces, and types of work performed. The list of personnel subject to medical examinations is reviewed annually.
Information on periodic inspections of employees is stored within the organization for three years.

Responsibility
If an employer does not monitor the health of his subordinates and does not conduct regular medical examinations, he commits an administrative offense under subsection 3 of Art. 5.27.1 Code of Administrative Offences. The amount of penalties under this article will be:
- for an official and for organizations operating without forming a legal entity - from 15 to 25 thousand rubles;
- for a legal entity – from 120 to 130 thousand rubles.
In case of failure to conduct examinations of vehicle drivers, liability is possible under Article 11.32 of the Code of Administrative Offenses. The amount of penalties in this case will be:
- for responsible persons – from 2 to 3 thousand rubles;
- for legal entities and organizations not declared as legal entities - from 30 to 50 thousand rubles.
How to make a list of contingents
This is the first stage of preparation for a medical examination, and the medical commission focuses on it when drawing up criteria for assessing the health of employees. Based on the contents of the Order, in order to issue it, it is necessary to identify harmful production factors and types of work in accordance with the staffing schedule.
The algorithm of actions when compiling a contingent of workers subject to periodic medical examinations is as follows:
- It is recommended to first draw up a list of the contingent, and then by name, but many do the opposite. To facilitate the procedure, personnel officers often first create a list of all employees of the enterprise by name, and on its basis they compile a list of persons subject to periodic medical examinations.
- A list of all professions in the organization is compiled, focusing on Appendix No. 2 to Order No. 302n.
- For each profession, harmful factors and work are indicated in accordance with Appendix No. 1 to the Order in order to determine the list of professions subject to periodic medical examinations. To determine which factors need to be specified for each specific job, you can use the results of workplace certification, the production control program, or last year's contingents. If you have employees for whom there are no harmful factors and jobs, the corresponding cells in the contingents for this profession should be left empty. Such workers do not need to be sent for a medical examination.
- Approval by the head of the organization.
- Sending the list to Rospotrebnadzor at the actual location of the organization within 10 days after its approval by the employer (clause 21 of the Order).
The procedure for creating a list of contingents
The document refers to the internal forms of the organization and is drawn up in any form, taking into account the norms provided for by law. At the same time, the employer is responsible for financing and conducting medical examinations, and failure to carry out the mandatory procedure entails administrative penalties.
The following categories of persons are subject to inclusion in the contingent for medical examinations:
- Workers exposed to hazardous production factors with a labor class higher than 3.1. The main confirmation when classifying working conditions as dangerous is the current results of workplace certification and assessment of working conditions.
- Employees employed in positions in accordance with Appendix No. 2 of Order 302n. In this case, the labor class does not matter. For example, these are employees of child care institutions, food industry enterprises, catering organizations, water supply facilities, etc.

Important! After January 1, 2014, workplace certification was replaced by a special one. assessment of working conditions (ASW), but the regulatory formulation has so far remained the same.
Who should undergo a medical examination
The Labor Code and the Order also contain a list of professions subject to periodic medical examinations. These are employees under the age of 21, as well as those who work:
- in harmful and dangerous production;
- at work at heights;
- in transport (drivers, machinists, etc.);
- in the field of catering;
- in educational and children's institutions;
- in pharmaceutical, medical and sanitary institutions (spas, saunas and laundries);
- in beauty salons that provide hairdressing and cosmetology services;
- in the cleaning service;
- in the field of water supply;
- at livestock enterprises and poultry farms.
The list is open, and the employer, in agreement with subordinates, can organize regular medical examinations for any other professionals.
The legislator also establishes the frequency of such medical examinations, which depends on the working conditions and is detailed in Appendix No. 1 to the Order. On average, the listed categories of workers should be examined at least once every 1-2 years.
How to quickly draw up lists of employees for periodic medical examinations according to Order 302N?
You say, “What is the question?” Order 302N of the Ministry of Health and Social Development came out a long time ago, everyone has already read it, studied it and put it into practice. A sea of articles has been written, thousands of seminars have been held, and yet questions of efficiency and effectiveness remain. The main problem is how to take everything into account!

You can use the occupational directory to determine what harmful factors are traditionally present among these employees. Can be assessed using production control data. And if a special assessment of working conditions was carried out, then it would be even more interesting. For a medical examination, it is necessary to take into account the identified harmful factors identified by the expert and indicated in the working conditions card, and perhaps also those present at the workplace based on the results of production control. There are disagreements in the interpretation of regulatory documents of 2 departments of the Ministry of Health and Social Development and the Ministry of Labor.
Compiling a list of contingents is not a trivial task; it requires tracking changes, and the list of contingents is a “living” document that must be constantly updated. Store it somewhere and work with it somewhere, of course it’s better if in a specialized program. Drawing up lists of employees to undergo a medical examination
If the contingent list is ready, then half the work is already done. It is necessary to identify workers employed in these professions using this list.
There is another option: take last year’s old lists of completed medical examinations and add there the workers who came, subtract those who quit or transferred. At first glance, this is simpler, but you still have to do a painstaking reconciliation, and it’s not a fact that if working conditions change, everyone needs to be checked again at a medical institution for the same factors. As we see, there are too many conventions. And that's not all. Order of the Ministry of Health and Social Development 302N establishes different frequencies of medical examinations: once a year and once every 2 years. This means that there is no need to go through extra doctors every year, but you need to take into account when you had your last medical examination.
Make 2 lists, last year’s and the year before last. It is necessary to exclude unnecessary doctor's appointments and studies. According to my estimates, every fifth doctor and test can be excluded, and this is already 20% of the cost of the medical examination! It is worth mentioning that at the same time it is necessary to calculate the length of work in hazardous conditions, take into account special cases of contingents, youth under 21 years of age, women over 40 years of age and those undergoing examination in professional centers. pathology. Now about the timing. Traditionally, medical examinations are carried out in the unit once a year. We usually do this: we made a list and sent everyone, including those who underwent a medical examination just a month or two ago when applying for a job. This is done so that you don’t have to look for it later and don’t take it into account separately. The order stipulates that it should be carried out annually, but does not mean at one time. It is more rational, of course, to take into account the needs for medical examinations and conduct them when and according to those factors when it is really required. Likewise, it is better and cheaper for an enterprise when not everyone leaves at once, but one by one, quarterly. It is not difficult to calculate that with a high staff turnover at an enterprise, switching to such a “sliding” frequency will help save up to 25% of the cost of a medical examination. This cannot be done manually, without a customized program.
Referral for medical examination
Order of the Ministry of Health and Social Development 302N stipulates that after agreeing on the lists, it is necessary to issue referrals for a medical examination in the approved form, before this it is necessary to agree on a schedule for the medical examination. This task already seems simple, but there is a need to prepare the following administrative documents and forms:
- An order to conduct a periodic medical examination, with the order to be familiarized with signature.
- The list of persons to undergo a medical examination must be approved.
- The calendar plan for conducting a medical examination must be coordinated with the departments and the medical institution, and be sure to familiarize the employees.
- Provide each employee with a referral to undergo a medical examination.

Medical examination results
If an employee has passed a medical examination, then he will receive a health passport, and we will learn about it only after we receive the final report from the medical institution. What if you didn't pass? This is where additional work arises. We must suspend everyone who did not deign to visit the doctors from work, make an order in the form of an order for suspension from work in accordance with the requirement of Article 72 of the Labor Code of the Russian Federation. Don't let you go to work, don't pay wages. And this must be done quickly.
If a conclusion on the employee is not given or he is sent for further examination, then we can assume that for now he has no contraindications, but we must take into account that an additional decision will be made by the medical commission on him.
